I also dislike when all the characters sound the same. Or when none of the fights carry any risk anymore, but the story hasn't adapted to this. Also, when side characters don't have their own goals, preferences, etc, that colour their words, how they receive the protagonist's actions, etc.
Things I find offputting are when the protagonist is very passive or where it seems like the author is very reluctant to deviate from canon's rails and risk things by changing the world. E.g. in Paranoid Mage, I liked that the world changed and the protag made that happen
