Yeah Sarutobi was aware of what was going on and supported Ochi. He didn't know the scope of it and obviously didn't know he was experimenting on Konoha civilians and ninja's, which to him was treason and the reason Ochi was banished, but his hands are not completely clean of the situation and he feels some guilt too at how it all played out.

My thinking on the relationship between Tsuande and Ochi is that they used to be a lot closer, but time and the crime Ochi committed has strained that alot. Still every year or so the two sit down at a coffee shop or a bar and have a few drinks as they share secrets, theorize new medical techniques, and laugh at old stories, both wondering when it'll be the last visit.

Ochi's doing what canon Ochi was doing. Going around, doing favors and work for various groups and organizations to build up the money and resources to keep doing her research into immortality. With a much greater focus on the Senju than in Canon where he was obsessed with the Uchiha.
