    I interpreted this as less Twilight's status being an active threat to Velvet so much as how Omake Velvet let her self-image be tied into the idea of being a "protector" to the point that she felt forced to get a new Twilight so she wouldn't have to be reminded of her own fallibility.

    Velvet seems to be aware of the Therapy group to some degree, and encouraged Softy to follow her heart as to what to do with it. However, she doesn't seem aware of what it's turned into since then.
