I wasn't around when that happened, but I distinctly remember reading in discussions that if we had a fitting Sacrament the choice wouldn't be as bad.
And look, here we do get a Sacrament in return!

So he is somewhere out there doing whatever Biedde wanted him to do, but he isn't going to return and help Biedde's cult against us, if that cult even exists.

Let's be real: we will want to befriend Smiles. And it would like 90% require becoming enemies with Biedde
Best case scenario, we do it the turn before All In. If we give Biedde enough time he can prepare contingency plans...

But Names are generally stuck in the Mansus, unless summoned. Master could converse with members of the cult, but not get out in the Wake himself before we summoned him.
So we better not let him stay in the Wake and build up this army
He can't subvert anybody if he is stuck in the Mansus, and we likely can neutralize most if not all of the ponies he have subverted if we turn against him now.
I would also note that if we don't take Smiles' sacrament right here and now, later it will likely become harder.
At the very least we are likely to have a challenge on the level of inflicting a wound on Biedde. Which, as you could see, was a very close call for us this time.
That would almost certainly happen.
The only way to maybe get this Sacrament without picking a side is to kill a Worthy Enemy. Even then, it is not unlikely we'll be forced to pick a side.
And it is such a huge delay we are unlikely to do it ever until the end of the quest.
I really don't like that with the coward's way out we basically lose this AP and one wound for nothing(yes, we got to know a new Name... Which we will summon a few turns later), while delaying a Sacrament enough to be very likely to not get it at all
With Smiles fighting whatever Biedde managed...
Except we are getting an extra Name at best next turn, more likely a turn after that or even later, and need to pay more for the upkeep of that Name than if we take the offer. While we can get Sacrament right now.
Worm museum does not hold "a" Worm. It holds multiple Worms, always dying but never quite dead.
The Colonel is the main guardian of the museum, and normally the Lionsmith shouldn't challenge him there but it seems the times have changed.

Natures of the Colonel are: to be blind, to be scarred, to cannot be denied.
Natures of the Lionsmith are: to be stronger, to be seamless, to make monsters.
Colonel doesn't commit to duties by default, Lionsmith doesn't necessarily betray.

There is one other organization which could summon Biedde if we let them grow: the Eclipse.
Smiles seems to only be reachable through Sacrament with Biedde. Biedde, however, stands guard over the Worm Museum, Eclipse could reach him if we let them...
