Sometimes, yeah.

But there are legit some stories that actually explore the differences between Step vs. Actual incest and the potential consequences.

This one fic I read had the poor kid lose his mother to a car accident with the body missing. (Presumed Dead).

Father remarries, and shortly thereafter dies, leaving everything to the Son and his two sisters, because he didn't change his will just yet to include the Step Mother and two twin Step Sisters.

Sexy stuff happens with the step sisters, and they officially start dating him, causing quite the stir. Legal questions come into play, but since marriage isn't being discussed that isn't an issue.

Actual sisters get tired of walking on of the three having sex, and older sister gives it a shot while frustrated and the guy being asleep. Devolves from there when he wakes up. Younger sister gets into it because of bath time and she accidentally knocks him out while trying to get in on the action.

Step mother gets in on it by basically walking in on an orgy several times and joins in after a while.

Mother was actually alive, and was being cared for by an elderly couple in the countryside, slowing getting back her memories.

She comes back, and thinks that the guy (now an adult) is her husband since they look nearly identical. She fucks him while the guy has no idea who she is. He thinks she's his father's mistress back in the day or something.
